Going insane isn't death, and is perfectly acceptable for genjutsu. However that involves neural damage, still making if kinjutsu (only if it's permanate insanity though)

this also dwells into choosing to never be hit by a ninjutsu in your entire ninja career. Sometimes, death is better then some other possibilities. But it is still a possibility. If I choose to do the basement thing, eventually they will have to choose death.

It wouldn't need to be Kin, if it is one of 3+ options. And the choice would have to make sense. Just like being able to know your in a Genjutsu, if it doesn't make sense, it isn't allowed to be escaped/avoided.

If I was a cardiac victim who just got out of the hospital from a heart attack induced by fear. If it should happen again shortly after, I can't just be A-okay.

Heart Attack is serious, but one can live without going to a hospital immediately from one. Though obviously should be rushed to one. It depends on severity of heart attack.

Or perhaps we should have a Mod choose.... Cool

Acute stress can cause heart failure, however, its not up to my genjutsu to decide whether or not the person has heart failure. It is up to the user. >_>

Though for some reason i think 2 ranks beneath user is to much again just like the other jutsu from the other guy. I still think this is a possibility for higher ranked as well, though obviously less likely or takes longer.

If you see enough death, you become used to it. And death might not scare you as much as before. If you become used to be tortured, then torture might not be as bad for you anymore. What hurts one person might not hurt another, but might kill someone else.

It may cause heart failure, but that is only one factor.
